SERVICE GUIDLINES
DENTAL
• Pregnant patients must obtain clearance from their OB-GYN before receiving dental service
Vision
• Your appointment includes a comprehensive eye screening using advanced equipment inside our Mobile POD.
• Contact lens prescriptions are not included in this service.
Audiology Hearing Test
• Please arrive a few minutes before your scheduled appointment.
• The test will be conducted inside our Mobile POD, which is designed to reduce external noise.
• Noise-canceling headphones will be provided for your comfort.
Biometrics & Instant Lab Screening
• Fast for eight hours before your appointment to help ensure accurate results.
• Wear a short-sleeved shirt or polo for easier access during the screening.
Musculoskeletal Assessment
• You will receive a MyMedicalHUB invitation a few days before your appointment.
• Register at accesscare.mypainlog.ai.
• Complete the Phase 1 questionnaire before your scheduled visit.
• On the day of your appointment, wear light-colored athletic clothing.
• Avoid black or dark clothing and dresses during Phase 2 biometric and orthopedic testing.
• Your assessment report will be emailed to you within a few days after the appointment.
FLUSHOTS
• Wear a short-sleeved shirt or clothing that allows easy access to your upper arm.
• Inform the healthcare provider about any allergies, previous reactions to vaccines, or relevant medical conditions before receiving your flu shot.
• Let the healthcare provider know if you are currently feeling unwell or have a fever.
• Participants who are pregnant or have concerns about vaccination should speak with their healthcare provider before their appointment.
• Bring a valid photo ID and insurance card to your scheduled appointment.
• Plan to remain at the vaccination area for approximately 15 minutes after receiving your flu shot for routine observation.
MAMMOGRAPHY
• Mammography services are available only to patients aged 40 years and above. This age requirement follows recommended screening guidelines and ensures that services are provided to those most commonly advised to undergo routine breast cancer screening.
• Patients must not be undergoing treatment for breast cancer or have been diagnosed with breast cancer within the past two years.
• Patients must not have any new breast health concerns, such as a palpable lump, skin changes, or nipple discharge.
• Patients must not be currently breastfeeding or have breastfed within the last six months.
• Patients must not have undergone a mammogram within the last 12 months.
• Participants must bring a valid photo ID and insurance card to their scheduled appointment.
